My thoughts and concepts of trends in nursing have changed in different areas of nursing. I have learned that organizations such as the ANA are active in making changes for nurses and that nurses can also advocate and join the ANA.  I have also learned about new trends in nursing such as the demand for nurses to get their DNP, and how the mental health of health care workers will be a priority. With the many issues we face in nursing, I have learned that we can also speak to local legislative offices by sending letters, face-to-face appointments, and phone calls. The safe-patient ratio is still an issue that nurses are facing but I have learned that there are ways to battle that such as speaking to team leaders and also learning about the rules and regulations about staffing.  I think a  nurse-driven staffing committee that develops staffing plans that take the needs of the patient population into account and complement the knowledge and expertise of the staff should be required in hospitals. The American Nurses Association is in favor of laws and regulations at the state and federal levels that permit flexible nurse staffing plans and provide nurses the authority to design staffing plans that are unique to each unit. Direct care nurses are well-suited to contribute to the creation of staffing plans since optimal staffing is much more than just a matter of numbers.

Vee is a 26-year-old African-American woman who presents with a history of non-suicidal self-injury, specifically cutting her arms and legs, since she was a teenager. She has made two suicide attempts by overdosing on prescribed medications, one as a teenager and one six months ago; she also reports chronic suicidal ideation, explaining that it gives her relief to think about suicide as a “way out.”

When she is stressed, Vee says that she often “zones out,” even in the middle of conversations or while at work. She states, “I don’t know who Vee really is,” and describes a longstanding pattern of changing her hobbies, style of clothing, and sometimes even her job based on who is in her social group. At times, she thinks that her partner is “the best thing that’s ever happened to me” and will impulsively buy him lavish gifts, send caring text messages, and the like; however, at other times she admits to thinking “I can’t stand him,” and will ignore or lash out at him, including yelling or throwing things. Immediately after doing so, she reports feeling regret and panic at the thought of him leaving her. Vee reports that, before she began dating her current partner, she sometimes engaged in sexual activity with multiple people per week, often with partners whom she did not know.

different cultures so that you may reflect on and analyze your
intercultural communication competence as well as the
importance of cultural diversity awareness. As part of your
analysis, you should apply concepts from your studies.

Dimensions of Culture, as presented by Communication in the
Real World (2016), and how they affect intercultural

C.Z. is a 20-year-old Caucasian male who is in his second year of college. He is seeking treatment due to persistent fears that campus security and the local police are tracking and surveilling him. He cites occasional lags in his internet speed as evidence that surveillance devices are interfering with his electronics. His intense anxiety about this has begun getting in the way of his ability to complete schoolwork, and his friends are concerned – he says they have told him, “you’re not making sense.”

C.Z. occasionally laughs abruptly and inappropriately and sometimes stops speaking mid-sentence, looking off in the distance as though he sees or hears something. He expresses concern about electronics in the room (phone, computer) potentially being monitored and asks repeatedly about patient confidentiality, stating that he wants to be sure the police won’t be informed about his treatment. His beliefs are fixed, and if they are challenged, his tone becomes hostile.

Creating the Optimal Clinical Environment

Nurse educators are responsible for both didactic and clinical education. Much thought and planning goes into determining clinical content and clinical placements.  Before you can negotiate clinical partnerships, you must decide on the type of clinical experiences students need and where they can get them.  After completing this week’s reading, and reflecting on your experiences as a student, consider how an educator should go about making these kinds of decisions. 

Initial Post: Use these prompts to think about the topic and share your ideas.

  • How would things like active learning, action and reflection, and higher level thinking influence your decisions about student placement (i.e. where would you place new students vs. senior students and why)?
  • Scheduling students on the same floor for longer periods of time allows students to adjust and feel comfortable in the learning environment.  On the other hand, more frequent changes may provide a greater variety of clinical experiences.  As an educator, how do you choose?  Which do you prefer? Which do you think students prefer? Support your answers with information about best practices.
  • What balance of clinical observation vs. hands on care is optimal and why?
  • Who should have the greater role in guiding and supervising students on a clinical floor, the instructor or staff nurses/preceptors? How might this be negotiated/configured.
