Population health is shaped by the social and economic conditions in which we are born, live, and work. Statistics indicate that many of us are one paycheck away from a financial crisis, especially since past recessions and the current state of inflation. In this module, you will explore the impact of poverty on health. In your professional roles and perhaps your own personal lives, you have all been exposed to the issues to some extent. The intent of this module will be to explore in more detail the prevalence of poverty, its impact on health, and barriers to attaining healthy outcomes.

Current National Healthcare Issue and its Impact on the Work Setting

            One of the most significant healthcare issues to date is the lack of insurance coverage for patients and the continued high costs of healthcare.  As a whole society needs healthcare, but fewer can afford it even with working full-time jobs. Here in the U.S., our method for paying for healthcare is where patients pay a fee for the services provided. When doing this, it devalues the primary care aspect of healthcare and supports a more volumed-based system so that the providers can increase the monetary gain for a payment model system (Park et al., 2018). There is no consistent way to make things perfect across the board, but there are ways to help such as bundling care and treatments and implementing monthly payments (Park et al., 2018). The social determinant that most affects this issue is the ability to have access to quality healthcare. Multiple inequalities affect patients in healthcare such as ethnic groups, social status, and economic (Broome, M., & Marshall, E. S.,2021). In 2020, the United States healthcare costs reached 4.1 trillion dollars (AAMC, 2023). “Nearly one in five Americans has medical debt, and affordability is still an issue for a large proportion of the population, whether uninsured or insured, which suggests that policymakers should focus on patients’ costs.” (Association of American Medical Colleagues, 2023). With that number of Americans being in debt due to healthcare, we need to focus on how to change the way we are billing our services as providers, offering insurance to the public as a country, and a way to offer more affordable services.

How the National Healthcare Issue Affects the Work Setting

In the clinic where I work our patients often must reschedule the procedures that they are scheduled for their pain management treatments due to insurance denying them. They will either pay out of pocket or reschedule. This impacts the practice as a whole: the nursing staff must rearrange the whole schedule, and change the documentation, and the patients will miss their much-needed treatments. Our case manager then works closely with the insurance companies to see what we can do to get the patient’s procedures approved. It has often taken up to 6 months to get just one injection approved. This process often makes the patient feel depressed and angered and leaves the staff feeling hopeless.

1. What staffing model is implemented at the facility where you work?  What would you recommend for modifying the staffing model where you work, what is you recommendation based on and how would you implement the change?  (Support your thoughts/ideas with academic literature OR required readings) 

The staffing model the facility where I work is a nursing home. We use a census as our staffing model. On my floor there is a total of 23 rooms and 2 clients per room. Most of the time we are short staffed, and as a result there are only 2 nurses and we each have 23 patients sometimes less if patients get discharged. Per facility each nurse is suppose to have 9 patients. When dividing patients among nurses I think it is important to take into account patient acuity, some patients require more intense care than others. Meaning if the patients are equally divided the workload may not be. I believe that it is important to talk to your coworkers about this concern to prevent placing patients at risk and create a positive environment where the workload is shared. A lot of times coworkers are not willing to change assignments and that is where I think the unit manager should step in. To fix this problem more staff should be hired to keep the 9:1 ratio and the unit nurse should review assignments, before they are given. Most nurses experience substantial fatigue, with high acuity patients having an overall greater impact. We recommend that NPAs shall contain fewer high acuity patients than lower acuity patients. Additionally, The OFER acute fatigue scale (OPER-AF) suggests that assignments should contain no more than five patients to mitigate fatigue (Mollica and Schwerha, 2021).

Betty, the chief nursing officer, had to make a decision about buying 120 new hospital beds for patient rooms. After she interviewed nurse mangers at the units where the beds were going to be placed, Betty compiled her findings and decided to contact a well-known equipment company to obtain prices and contracts. The equipment company’s executive salesperson, Jim, discussed options at length with her and invited her and her significant other to an upcoming all-expenses-paid lavish retreat at a five-star hotel in Hawaii to see demonstrations of the beds and to hear a comprehensive sales pitch.
