EHRs Benefits and Drawbacks

Preparing the Discussion

Post a written response in the discussion forum to EACH threaded discussion topic:

1. As discussed in the lesson and assigned reading for this week, EHRs provide both benefits and drawbacks. Create a “Pros” versus “Cons” table and include at least 3 items for each list. Next to each item, provide a brief rationale as to why you selected to include it on the respective list.

2. Refer to the Stage 3 objectives for Meaningful Use located in this week’s lesson under the heading Meaningful Use and the HITECH Act. Select two objectives to research further. In your own words, provide a brief discussion as to how the objective may impact your role as an APN in clinical practice.  

Adhere to the following guidelines regarding quality for the threaded discussions in Canvas:

· Application of Course Knowledge: Demonstrate the ability to analyze, synthesize, and/or apply principles and concepts learned in the course lesson and outside readings.

· Scholarliness and Scholarly Sources: Demonstrates achievement of scholarly inquiry for professional and academic decisions using valid, relevant, and reliable outside scholarly source to contribute to the discussion thread. 

· Writing Mechanics: Grammar, spelling, syntax, and punctuation are accurate. In-text and reference citations should be formatted using correct APA guidelines.

· Direct Quotes: Good writing calls for the limited use of direct quotes. Direct quotes in discussions are to be limited to one short quotation (not to exceed 15 words). The quote must add substantively to the discussion. Points will be deducted under the grammar, syntax, APA category. 

For each threaded discussion per week, the student will select no less than TWO scholarly sources to support the initial discussion post.

Scholarly Sources: Only scholarly sources are acceptable for citation and reference in this course. These include peer-reviewed publications, government reports, or sources written by a professional or scholar in the field. The textbooks and lessons are NOT considered to be outside scholarly sources. For the threaded discussions and reflection posts, reputable internet sources such as websites by government agencies (URL ends in .gov) and respected organizations (often ends in .org) can be counted as scholarly sources. The best outside scholarly source to use is a peer-reviewed nursing journal.  You are encouraged to use the Chamberlain library and search one of the available databases for a peer-reviewed journal article.  The following sources should not be used: Wikipedia, Wikis, or blogs.  These websites are not considered scholarly as anyone can add to these. Please be aware that .com websites can vary in scholarship and quality.  For example, the American Heart Association is a .com site with scholarship and quality.  It is the responsibility of the student to determine the scholarship and quality of any .com site.  Ask your instructor before using any site if you are unsure. Points will be deducted from the rubric if the site does not demonstrate scholarship or quality. Current outside scholarly sources must be published with the last 5 years.  Instructor permission must be obtained BEFORE the assignment is due if using a source that is older than 5 years.

EXPLORING MIDDLE RANGE THEORIES AND FRAMING PRACTICE ISSUES

  • Analyze your nursing practice (psychiatric) for issues of particular interest or concern to you. Identify one issue as the focus of your application of theory to practice.
  • Review the Learning Resources to identify specific middle range theories that may apply to your practice issue. (check below)
  • Choose at least two middle range theories that might be most relevant and valuable in addressing your practice issue.
  • Search the Walden University Library for scholarly articles that address application of middle range theories to practice issues.
  • Consider how to frame your focus practice issue in terms of the middle range theories that you have selected.

Identifying and justifying appropriate Study Designs · 

Step 1:  A not-for-profit organization called Epidemiologists Without Borders has reached out to student researchers studying epidemiology at Florida National University. The organization has asked for support in the design of three research projects they plan to carry-out in several of the communities where they currently work. Read these three case studies summarized below.

NOTE: You may substitute one of the case-studies outlined below for a case study that is relevant to your own community. If you choose to do so, generate a case-study regarding a public health problem that is relevant and requires epidemiological research. Include the case study when you submit your assignment.

Case study 1:

Vaginal cancer is a rare disease that can often be cured when detected in its early stages. Vaginal cancer occurs more often in women over the age of 50 but can occur at any age. There are usually no symptoms for vaginal cancer. Epidemiologists Without Borders has been awarded a large grant to study the relationship between vaginal cancer and prior exposures to any risk factor among women in a South African city.  

Case study 2:

Epidemiologists Without Borders has learned that a large group of employees who have worked in a specific building (building A) of a very large corporation for more than 25 years appear to have high cancer incidence rates. The corporation has a total of 3 office buildings (buildings A, B, and C). Epidemiologists Without Borders would like to conduct research to determine if the rates of cancer incidence among those workers from building A are in fact higher than expected.

Case study 3:

Epidemiologists Without Borders supports the rights to life, health, and dignity for people who use illicit drugs. The organization is concerned about the high rates of HIV infection it has observed in one particular community and suspects that there are many risk factors possibly contributing to this high HIV risk. Currently, the data in this community with respect to the exact HIV incidence rate among injection drug users is of poor quality but estimates indicate that the prevalence is as high as 10-20%. Access to HIV testing is also low in this population. Additional challenges include limited resources, and the difficulty in recruiting people for testing due to their high mobility and hidden social networks. 

Epidemiologists without Borders would like to identify potential risk factors for contracting HIV among injection drug users in this community so that it can develop appropriate interventions.

Step 2: Select the most appropriate study design for each of the three case-studies proposed by Epidemiologists Without Borders (or for the one relevant in your community) based on what you have learned in this Week’s Lectures, Readings, PPT’s and modules. 

Step 3Write a letter responding to Epidemiologists Without Borders outlining the study designs that you recommend. Justify why the study designs you selected are appropriate for each of the three case-studies. Your response must also outline at least one benefit and one limitation for each of the study designs you have recommended for each case-study. Your letter to Epidemiologist Without Borders should be clear and concise and contain no more than 450-550 words.

3.  As a reminder, all discussion posts must be minimum 350-550 words, references must be cited in APA format 7th Edition, and must include minimum of 3 scholarly resources published within the past 5-7 years (not part of the classroom coursework).  

Fibrinolytic agents

 

Discussion:

Fibrinolytic agents present the only available pharmacotherapy able to dissolve a thrombus and open occluded vessels.  Despite the worldwide experience with fibrinolytic therapy, there are limitations that must be considered. 

As with all other management strategies for Acute Coronary Syndrome (ACS), contraindications for fibrinolytics in patients diagnosed with an ST-elevated myocardial infarction (STEMI) must be considered.  

Discuss in detail the absolute and relative contraindications for fibrinolytic therapy in STEMI.

There are two parts to complete your assignment for this week. Please review the instructions carefully.

Part 1:

 Access the Institute for Healthcare Improvement's Quality Improvement Essentials Toolkit at the link in your Weekly Resources. Once at the link, you may be prompted to create an account to access the IHI Toolkit and the Driver Diagram. By creating an account, you will have access to many resources provided by the Institute for Healthcare Improvement.

 Download the template and save the template to your computer with your standard file-naming protocol. Be sure this file name includes the words Driver Diagram. Note: When downloading the template using the Chrome browser and the download arrow, a message appears asking if the form is to be saved “with my changes” or “without my changes.” Be sure to save with your changes even if you haven't made any changes. This action will save the template with your work, otherwise, it will not be saved.

 Close your browser after saving the form.

 Find the file you just saved on your computer and open it up.

 Read the information about Drivers in the form and review the example in the template.

An example of how the Driver Diagram should look as you begin filling in the template is shown in Figure 8. In this example, there are Primary Drivers listed which you may use in your diagram or you can select different Primary Drivers.

Part 2:

Once you have completed and saved your Driver Diagram, you will prepare a Perspective Paper discussing your rationale for selecting the Primary Drivers, the Secondary Drivers, and the specific ideas to move toward creating an organizational culture of person-centered quality health care. Keep in mind, your perspective will provide a reasoned argument why you selected the drivers and ideas.
Hint: A good method to use when preparing a perspective paper is the A-B-C-D-E method; answer the question why the selections were made, back up your decisions, critique your decisions, defend your decisions, and evaluate your decision against other alternatives.  

Be sure your Perspective Paper contains an Introductory section, a section with the Body of your report, and a Summary Section. In addition, you will submit your Driver Diagram in one of two ways. You may either take a screenshot of your completed Driver Diagram, paste it into a new Word document, crop it as seen in Figure 8, then right-click on the cropped area and select Save as Picture. Save the cropped area as a picture on your computer, giving it the same file name and the original Driver Diagram. Insert your saved picture of your Driver Diagram in your report as an Appendix. Enlarge the picture so it fills the Appendix page to the page margins. Be sure to consult APA formatting guidelines when using an Appendix. 

Alternately, in lieu of the Appendix page, you can upload your Driver Diagram as a separate document when you submit your Perspective Paper. Be sure to refer to the separate Driver Diagram document in your Summary Section.    

Part 1:
The Driver Diagram is one page if used as the Appendix in the Perspective Paper. If uploaded as a separate document, your completed Driver Diagram template is 1 page of the 4-page IHI document.  

Part 2: The Perspective Paper is a minimum of 3 pages, not including the title page, reference page, and Appendix page if used.

References: Include a minimum of 3 peer-reviewed resources. Other scholarly and credible sources may also be used as supplemental support.
