Eleanor Thompson, a 72-year-old woman, has been battling an aggressive form of ovarian cancer for the past three years. Despite undergoing multiple rounds of chemotherapy and radiation, her condition has deteriorated, and she is now experiencing severe pain and suffering. Eleanor’s oncologist, Dr. Samuel Martinez, has presented the family with the option of another experimental treatment, but he has also suggested considering transitioning to palliative care. Eleanor’s family is deeply divided on the decision. Her eldest daughter, Maria, believes that every possible treatment should be pursued, holding onto hope for a miracle. She argues that her mother has always been a fighter and would want to continue the battle against her illness. On the other hand, Eleanor’s son, Michael, feels that his mother has suffered enough. He believes that transitioning to palliative care would allow her to spend her remaining days in comfort and peace. Caught in the middle is Eleanor’s husband of 50 years, Robert. He is torn between wanting to honor his wife’s fighting spirit, desiring to ease her pain, and the painful finality of either decision. The healthcare team must navigate a complex ethical landscape. Beneficence, the principle of doing good, pushes the team to consider what would truly benefit Eleanor in terms of quality of life. Non-maleficence, or “do no harm,” raises questions about the potential suffering Eleanor might endure with further aggressive treatments. Lastly, the principle of justice requires the team to treat Eleanor fairly and equitably, ensuring her needs are met regardless of family disagreements.

COMPARING AND CONTRASTING PHARMACOLOGIC OPTIONS FOR THE TREATMENT OF GENERALIZED ANXIETY DISORDER

 

Psychological disorders, such as depression, bipolar, and anxiety disorders can present several complications for patients of all ages. These disorders affect patients physically and emotionally, potentially impacting judgment, school and/or job performance, and relationships with family and friends. Since these disorders have many drastic effects on patients’ lives, it is important for advanced practice nurses to effectively manage patient care. With patient factors and medical history in mind, it is the advanced practice nurse’s responsibility to ensure the safe and effective diagnosis, treatment, and education of patients with psychological disorders.

Generalized Anxiety Disorder is a psychological condition that affects 6.1 million Americans, or 3.1% of the US Population. Despite several treatment options, only 43.2% of those suffering from GAD receive treatment. This week you will review several different classes of medication used in the treatment of Generalized Anxiety Disorder. You will examine potential impacts of pharmacotherapeutics used in the treatment of GAD. Please focus your assignment on FDA approved indications when referring to different medication classes used in the treatment of GAD.

Discussion

Initial Post Instructions

Congratulations on successfully finishing the last of the A&P series! Over the course of the last eight weeks we’ve discussed the anatomy and physiology of the Digestive, Urinary, and Reproductive systems. We also discussed nutrition, metabolism, human development, and genetics. And since we began our study of the human body in BIOS251, we have continuously referred to a unifying theme in Anatomy and Physiology; that is, the relationship between structure and function. The idea that “form follows function” has been the foundation of our understanding of the human body and how it sustains life. This common thread throughout the A&P series allows us to look at the “big picture”.

If you remember from BIOS251, one of the fundamental principles of Anatomy and Physiology is the interdependence of all body systems and how the proper function of one system is intimately tied to the proper function of other organ systems, and ultimately the overall homeostasis of the human body.

For the Week 8 discussion of BIOS256, I would like you to go back in time. Think back to previous sessions and consider the various organs and organ systems that were discussed in BIOS251, BIOS252, and BIOS255. Choose one of the following organs or organ systems:

  • Integumentary system

For example, in week 1 and 2 we discussed the digestive system, and the importance of intrinsic factor to red blood cell production. One approach would be to discuss the connection between dietary absorption of intrinsic factor, its relationship to vitamin B12, and the maintenance of a healthy hematocrit.

Kel's case study analysis

  

Purpose: Analyze and apply critical thinking skills in the psychopathology of mental health patients and provide treatment and health promotion while applying evidence-based research.

Scenario:

Kel is a 42-year-old certified public accountant (CPA) who dreams each year that she will board a cruise ship the day after Tax Day and go somewhere, anywhere, except Portsmouth, Virginia. Each year the dream, like the ocean, ebbs and flows, but this year she is not even going to think about such a “ridiculous idea.” In fact, she does not even have the energy to dream; getting out of bed and preparing to go to work is simply too labor intensive. 

Each evening Kel retires to bed with a lack of energy to complete her normal tasks such as readying her clothes for work and making a lunch. She lacks the energy to shop in the evening; consequently, she eats mostly crackers and canned soup. She is not hungry, and her scale reflects this. She has lost 15 pounds over the last 2 months. She does not attend to her makeup or clothes; she finds both too taxing. The clothes she selects are drab and not ironed. At work she makes no effort to talk with her co-workers and does not initiate new contacts with clients. The normal work of filing taxes and writing reports, which she used to enjoy, are overwhelming, and she feels too disorganized to complete them. Telephone calls and e-mail messages from friends are ignored. Attendance at work is spotty.

Sue, her sister, becomes alarmed with Kel’s unanswered telephone calls and e-mails. Worried, she decides to visit her sister at home. She finds the apartment unclean and in disarray. Kel is unkempt, disheveled, and looks sad. Her voice is monotone and flat. Kel tells Sue that she feels “sad and hopeless. Nothing is ever going to change. I am a bad person and I can’t even do my work right. Although I sleep for many hours, I am still tired all the time.” Sue is alarmed at the changes in her sister and arranges for Kel to visit a health care worker at the medical clinic.

His history also includes being diagnosed with type 2 diabetic, high blood pressure, coronary artery disease, polycythemia, and atrial fibrillation. His father died of an MI at age 53. Both of his parents had type 2 diabetes. His mother dies of cancer. His paternal grandfather had 2 strokes, 0neyear apart, dying two weeks after the second stroke.

1. What types of extremity paralysis ( weaknesses) can occur in strokes and how do they differ? What type of weakness does this patient exhibit?

2. The lesion that caused the motor function problem in this patient is located on which side of the brain? Why does it occur in this manner?

3. Given the history of this patient, what type of stroke did this patient have,

ischemic, or hemorrhagic? Please provide your rationale.
