Assignment Overview:
 

The Week 3 Assignment is part of a scaffolded assignment and serves as the basis for creation of the Week 5 Assignment.  

For the Week 3 Assignment: Nursing Grand Rounds Presentation Part 1: Holistic Assessment, you will meet with and assess a selected participant and complete the accompanying Holistic Assessment Form.  In Week 5, you will complete the Nursing Grand Rounds Part 2: Presentation, which is a teaching presentation.

For the Week 5 Assignment, you will use the data collected in the Week 3 Assignment to create a PowerPoint presentation designed to educate your selected participant about an identified medical condition they are experiencing. The Week 3 Assignment data will be used as a foundation for creating the patient-centered teaching presentation due in Week 5.

Assignment Directions: 

· Identify an participant known to you who has a current medical health condition. The participant must be 18 years or older and legally capable of consenting to participate in this activity. The participant 
CANNOT be a family member or a client you have cared for in your professional nursing practice. Using a client or patient who was assigned to receive care from you by an employer, agency, facility, or any organization will result in earning a score of zero (0) points for the assignment.

· Be sure the participant is diagnosed with a medical health condition that meets the assignment requirements. The participant’s medical health condition cannot be congenital (i.e., present at birth). Instead, the medical health condition must have a pathophysiology. The medical condition also must be a primary health alteration (e.g., cyclic vomiting syndrome), and not simply a sign or symptom that is a manifestation of a primary medical disorder (e.g., hyponatremia, dehydration, nausea, vomiting, etc.).

· Complete the 

Holistic Assessment Form
 for the selected participant, then upload the completed form to the Dropbox by the end of Week 3. The form should be typed. The course faculty will review and grade the form for completion and accuracy and will approve the participant/medical condition you have selected as the focus for the Week 5 Assignment.

· After successfully completing the Week 3 Assignment and obtaining faculty approval of the selected participant/medical condition, follow the instructions for completing the Week 5 Assignment: Nursing Grand Rounds Part 2: Presentation.

Each year, 1.5 million Americans are diagnosed with diabetes (American Diabetes Association, 2019). If left untreated, diabetic patients are at risk for several alterations, including heart disease, stroke, kidney failure, neuropathy, and blindness. There are various methods for treating diabetes, many of which include some form of drug therapy. The type of diabetes as well as the patient’s behavior factors will impact treatment recommendations.

For this Discussion, you compare types of diabetes, including drug treatments for type 1, type 2, gestational, and juvenile diabetes.

• Select one type of diabetes to focus on for this Discussion.
• Consider one type of drug used to treat the type of diabetes you selected, including proper preparation and administration of this drug. Then, reflect on dietary considerations related to treatment.
• Think about the short-term and long-term impact of the diabetes you selected on patients, including effects of drug treatments.

 Post a brief explanation of the differences between the types of diabetes, including type 1, type 2, gestational, and juvenile diabetes. Describe one type of drug used to treat the type of diabetes you selected, including proper preparation and administration of this drug. Be sure to include dietary considerations related to treatment. Then, explain the short-term and long-term impact of this type of diabetes on patients. including effects of drug treatments. Develop a PowerPoint presentation that includes the following criteria:

• Slide 1: Title Page.
• Slide 2: SBIRT overview – Describe the history of SBIRT, the SBIRT process and how it is used in clinical practice settings using US research articles.
• Slide 3:  Case Presentation – Do not include patient identifying information but include demographics i.e. age, gender, race/ethnicity, chief complaint, social history, family psychiatric history, psychiatric history, and risk factors.
• Slide 4:  Screening Tool used – Describe the screening tool, validity (sensitivity and specificity), scoring information, and citation.  Your patient’s score and the interpretation of the patient’s score are required.
• Slide 5:  Brief Intervention – How was the Motivational Interviewing process applied and shared your client’s score and need for behavioral changes? Discuss the four stages of motivational interviewing and provide details of how the techniques were used.  A transcript of your encounter with the patient conducting the OARS technique is expected.  Discuss the roadblocks used or those avoided to obtain full points. Discuss the patient’s stage of change before the intervention was performed
• Slide 6:  Referral for Treatment – Describe 3 referrals for follow-up treatment plan.  Include the name, address, and telephone number of the local large organization.  Be specific with the department where necessary.
• Slide 7:  Evaluation of the process – Share the patient’s outcome and your evaluation of the SBIRT process.
• Slide 8:  References –5 references.  7th Edition APA format.  Include Screening Tool authors.

For the middle-aged adult, exercise can reduce the risk of various health problems. Choose two at-risk health issues that regular physical exercise and activity can help prevent and manage. Discuss the prevalence of each of these health problems in society today. Describe measures that you would take as a nurse to assist patients with health promotion measures to incorporate exercise and physical activity into their lives.
