Mark and Jacqueline have been married for 30 years. They have grown children who live in another state. Jacqueline’s mother has moved in with the couple because she has Alzheimer’s disease. Jacqueline is an only child and always promised her mother that she would care for her in her old age. Her mother is unaware of her surroundings and often calls out for her daughter Jackie when Jacqueline is in the room. Jacqueline reassures her mother that she is there to help, but to no avail.
Jacqueline is unable to visit her children on holidays because she must attend to her mother’s daily needs. She is reluctant to visit friends or even go out to a movie because of her mother’s care needs or because she is too tired. Even though she has eliminated most leisure activities with Mark, Jacqueline goes to bed at night with many of her caregiving tasks unfinished. She tries to visit with her mother during the day, but her mother rejects any contact with her daughter. Planning for the upcoming holidays seems impossible to Mark, because of his wife’s inability to focus on anything except her mother’s care.
Jacqueline has difficulty sleeping at night and is unable to discuss plans even a few days in advance. She is unable to visit friends and is reluctant to have friends visit because of the unpredictable behavior of her mother and her need to attend to the daily care.

This course provided foundational and introductory knowledge regarding informatics with an emphasis on developing an understanding of concepts of nursing informatics (NI) and becoming knowledgeable about the application of informatics in advanced practice. Reflect upon one NI competency relevant to APNs which you learned in this course. Provide a brief summary (100 words or less)
Evidenced-based practice in healthcare utilizes the current best and most relevant research, clinical experience, and patient/family valued preferences to deliver healthcare that not only improves patient experiences, but also provides better quality care for the patient and empowers clinicians, which leads to increased job satisfaction (Melnyk & Fineout-Overholt, 2023). While research clearly shows the benefits of evidence-based practice, there are a multitude of barriers that stand in the way of it becoming the standard of care such as: the lack of knowledge pertaining to evidence-based practice, lack of belief that this will result in more positive outcomes, overwhelming patient loads, and resistance to change just to name a few. Overcoming these barriers is necessary to move from practices based on outdated research in order to be able to provide better and safer healthcare.
Utilizing evidenced-based practice within the scope of my nursing license promotes lifelong learning and continuously improved nursing practice. My hospital has recently implemented bedside shift report for nurses, which has been shown to improve patient satisfaction and safety. Bedside shift report allows me to visualize both the patient and the surroundings in addition to communicating the plan of care to the patient. Initially, there was much resistance to changing the process of our shift report. Some nurses reported overwhelming patient loads (e.g. “I have to give report to 4 different nurses”) as a reason for which they struggled to do bedside report. After much encouragement from peers and leaders, the transition to bedside shift report has improved nursing accountability and teamwork. Visualizing all of my patients first thing in the morning allows me to ensure accuracy of the report and prioritize care.

Population health topics that rise to the presidential agenda level often highlight critical public health concerns. One such issue is mental health, a topic influenced significantly by social determinants (Smith, 2023). For the purpose of this discussion, we will focus on mental health and examine how two recent U.S. presidents, Barack Obama and Donald Trump, handled this issue.
Social Determinant Affecting Mental Health: One of the most influential social determinants impacting mental health is socioeconomic status. Poverty, income inequality, and lack of access to education and job opportunities can lead to increased stress, limited access to mental health services, and higher prevalence of mental health disorders.
Barack Obama's Approach: During his presidency, Barack Obama took several steps to address mental health issues. The Affordable Care Act (ACA), often referred to as Obamacare, expanded access to mental health services by mandating that insurance plans cover mental health treatment on par with physical health. This significantly reduced the financial barriers to mental health care, making it more accessible to a broader population. Additionally, Obama's administration increased funding for mental health research and initiatives, reducing the stigma associated with mental health problems and promoting mental health awareness (Johnson & Brown, 2016).
Donald Trump's Approach: Donald Trump's approach to mental health was less comprehensive than Obama's. His administration did not make significant strides in expanding mental health coverage or addressing socioeconomic factors influencing mental health. Although there were efforts to combat the opioid crisis, which has a strong connection to mental health, overall mental health policies were not a primary focus of his presidency (Smith & White, 2019).
What I Would Do Differently: To address the issue of mental health differently, I would propose a more holistic approach. This would involve:
1.
Addressing Social Determinants: Implement policies aimed at reducing income inequality, increasing access to quality education, and creating job opportunities. By improving socioeconomic conditions, we can reduce the stressors that contribute to mental health issues.
2.
Expanding Access to Mental Health Services: Continue the efforts initiated by the ACA to ensure that mental health services are easily accessible and affordable for all. This includes improving reimbursement rates for mental health providers to encourage more professionals to enter this field.
3.
Mental Health Education: Implement mental health education programs in schools and workplaces to reduce stigma and increase awareness. Encourage open conversations about mental health to foster a supportive environment.
4.
Investing in Research: Allocate resources to mental health research to better understand the causes of mental health disorders and develop more effective treatments and interventions (Brown & Johnson, 2022).
In conclusion, addressing mental health issues on the presidential agenda should involve a multi-pronged approach that considers social determinants, access to care, education, and awareness. This approach aims to create a healthier population with improved mental well-being.
