Module Pre-Assessments are your opportunity to practice applying module content before final submission of your Competency Assessment. For your final Competency Assessment, you will address scenarios related to the importance of nursing informatics in healthcare.

For your Module 3 Pre-Assessment, respond to the following prompts. These prompts match those in your final Assessment. Be sure to support your responses with evidence from scholarly literature.

Scenario:

A patient approaches you after her consultation with her physician. Her physician had reviewed her electronic health record with her during the consultation. The patient realized that some of the information in her EHR was incorrect. The patient asks you to make changes to this information. Is this possible? Why or why not? Write a 1-page response that includes the following:

  • Describe what you would tell the patient in this scenario regarding making changes to her electronic health record (EHR).
  • Explain your reasons for responding to the patient in the way you describe.
  • Be sure to reference any legal requirements related to EHRs.

Use APA format and must include mia minimum of 2 Scholarly Citations.

Soap notes will be uploaded to Moodle and put through TURN-It-In (anti-Plagiarism program)

Turn it in’ s Score must be less than 25% or will not be accepted for credit; it must be your own work and in your own words. You can resubmit, Final submission will be accepted if less than 25%. Copy-paste from websites or textbooks will not be accepted or tolerated and will receive a grade of 0 (zero) with no resubmissions allowed.

Must use the sample templates for your soap note. Keep this template for when you start clinicals. 

The use of templates is ok with regards to Turn it in, but the Patient History, CC, HPI, Assessment, and Plan should be of your own work and individualized to your made-up patient.

Scenario 1: Myocardial Infarction

CC: “I woke up this morning at 6 a.m. with numbness in my left arm and pain in my chest. It feels tight right here (mid-sternal).” “My dad had a heart attack when he was 56-years-old and I am scared because I am 56-years-old.” 

HPI: Patient is a 56-year-old Caucasian male who presents to Express Hospital  Emergency Department with a chief complaint of chest pain that radiates down his left arm. He states this started this morning and has been getting worse, pointing to the mid-sternal area, “it feels like an elephant is sitting on my chest and having a hard time breathing”. He rates the pain as 9/10. Nothing has made the pain better or worse. He denies any previous episode of chest pain. Denies nausea, or lightheadedness. Nitroglycerin 0.4 mg tablet sublingual x 1 which decreased pain to 7/10.

Lipid panel reveals Total Cholesterol 424 mg/dl, high density lipoprotein (HDL) 26 mg/dl, Low Density Lipoprotein (LDL) 166 mg/dl, Triglycerides 702 mg/dl, Very Low-Density Lipoprotein (VLDL) 64 mg/dl

His diagnosis is an acute inferior wall myocardial infarction.

Question:

Which cholesterol is considered the “good” cholesterol and what does it do?

EMERGING AND RE-EMERGING DISEASES FACT SHEET

Sir Francis Bacon said, “Knowledge is power.” This is most definitely true when it comes to diseases and how to prevent and treat them. As a nurse, you are charged with teaching patients how to prevent infectious diseases and what to do if they become infected. A powerful tool in your arsenal is the Fact Sheet. Usually comprised of one page of easy-to-read content, these leaflets can be distributed easily and can effectively inform your practice.

To prepare for this Assignment:

· Select one disease that is either emerging or re-emerging in the world today.

· Research the disease using both scholarly and non-scholarly resources.

· Determine your audience (patients, other nurses, schools, etc.) that you would want to share the Fact Sheet with.

· Select pieces of information that are appropriate for your audience.

Assignment Instructions:

Disease:  Meningococcal Disease

Audience: General Public and Healthcare Professionals in Behavioral Health Settings

· Submit: A 1- to 2-page Fact Sheet. 

· Indicate the audience on the Fact Sheet. 

· Give a brief history of the disease. 

· What are the implications of the spread of the disease? 

· How does one detect and prevent the spread of this disease? 

· How is this disease treated? 

· Your Fact Sheet should be visually stimulating, appropriate for your audience, and formatted with bullet points for easy reading. 

· Support your “facts” with at least 3 references.

  • Select one of the 2021 National Patient Safety Goals.
  • Share your thoughts about the opportunities and challenges related to achieving the goal in your work environment.

DEVELOPING A CULTURE OF EVIDENCE-BASED PRACTICE

To Prepare:

  • Review the Resources and reflect on the various strategies presented throughout the course that may be helpful in disseminating effective and widely cited EBP.
    • This may include: unit-level or organizational-level presentations, poster presentations, and podium presentations at organizational, local, regional, state, and national levels, as well as publication in peer-reviewed journals.
  • Reflect on which type of dissemination strategy you might use to communicate EBP.

Then Post a discussion post of at least two dissemination strategies you would be most inclined to use and explain why. Explain which dissemination strategies you would be least inclined to use and explain why. Identify at least two barriers you might encounter when using the dissemination strategies you are most inclined to use. Be specific and provide examples. Explain how you might overcome the barriers you identified. please provide at least 3 references
